Please fill out and return these forms for each child at least 2 weeks prior to the beginning of the camp session:

- Camp Medical History & Informed Release Form
If you have any difficulty downloading or printing these forms, please contact us at campearthconnection@gmail.com or (607) 844-3178 and we’ll be happy to send you the forms via e-mail or snail mail.
You have two options for returning the completed forms:
- Mail filled-out hard copies to Camp Earth Connection, 63 Hammond Hill Rd., Freeville, NY, 13068.
- If you’re able to fill them in electronically using Adobe Reader, return them by email to campearthconnection@gmail.com.
